If your office management software does not yet integrate with SteriRecall, a database can be populated, using your patient list, to get you started. We will provide you with a Confidentiality Commitment Agreement. If you are comfortable attaching a patient list, please do so. What type of spore incubator do you use? Please indicate 3M 390, 3M 490, 3M 490mini or Other.

Please indicate the duration of your spores.

Please list all of your sterilizers here, one per line, prioritized. For Midmarks, please indicate if it is an older ("beige") or a new ("white") model.

Please indicate all of the cycle types you run on each sterilizer here (eg, Midmark packs, Statim plastics, etc). One per line. This is used to customize your spore test software.

How many scanners do you require? That is, how many places do you intend to scan items?

Please provide a list of those who would verify cycles and spore tests, one name per line please.

Please attach a file (txt/doc/docx/pdf) with a list of up to 94 different items you sterilize. (A cassette/kit is one item.) This is used to produce your labels.

(NOTE: You do not need to provide a list if your Patient Management Software is one of the following: Tracker, ClearDent, MaxiDent, Live DDM, OpenDental.)
